Complete forms 16465 and SDE 0093, available on the
Forms
page. If the person applying is not the primary caregiver, custodial
parent, or legal guardian of the beneficiary (or their spouse),
you’ll also need to complete SDE 0093-B which is also
available on the Forms page.
Send the completed and signed form via the secure Message
Centre. Your request may take between 2 and 5 business days
to process.
Please note that if you wish to remove the former beneficiary, we
must first open a new family plan and transfer the individual account
to the family account before we can add the new beneficiary.
